What is a Mental Health Assessment?
A mental health assessment is a series of questions and tests conducted by a mental health professional to gauge the intensity of a mental health condition, evaluate the symptoms and also begin the process of creating a treatment plan.
The assessment is conducted like an interview, and we can do a physical, in-person assessment or an online assessment. Mental health assessments, also referred to as level of care assessments or intake assessments, are designed to help clinicians determine the most appropriate care for your needs and preferences.
The assessment will determine which service will best cater to the intensity of your symptoms. The assessment can take anywhere from a couple of minutes to an hour, depending on the extent of the assessment and the services that will be provided. During that time, you may discuss the following:
- The physical and emotional symptoms you might be experiencing
- Your personal history of mental health
- Your environmental factors
- Your family history of mental health
- Insurance or how the services will be covered
- Your most prevalent stressors
- Any co-occurring disorders
- Any medications you are currently taking

Clinical interview form
This is the most common form of mental assessment, particularly for individuals taking the assessment for the first time. The interview involves interview questions from our therapist. The questions may cover everything from your overall mood and reactions to basic life events that have occurred over a period of time.
The interview can also include questions about a person's background, including their experiences during childhood, to establish basic behavioural patterns.
To create a baseline, most of our questions focus on the period of months preceding the interview. This helps us note changes in your stimulus responses and behaviours during this time. The physicians can help spot potentially problematic mood swings or anxiety disorders.
It is normal to have a range of emotions, experiencing crippling feelings of anxiety, dread or fear over a period of months could indicate a mood disorder.
Physical exam
The physical exam is an assessment that helps to distinguish between psychological problems and issues that originate from your body.
Sometimes, the cause of a symptom might not be apparent until the physical exam is performed. This involves taking a brief family medical history and inquiring about any current medications and their specific details. Depending on the outcome of the physical exam, we may also decide to conduct additional lab tests to further investigate the symptoms.
Lab tests
This type of assessment encompasses a wide range of tests, including the collection of blood and urine samples from the patient. We may also consider medical imaging or monitoring equipment such as EEG, MRI or CT scans. These assessments can help to narrow the focus on your condition and advise on the best course of treatment.
Verbal tests
This involves verbal or written tests. They can be administered as multiple-choice surveys taken online or via a phone app. You may be asked to rate your emotional responses to certain situations or stimuli. At the same time, other tests focus on evoking responses based on imagery, which can take the form of drawings, photographs or a sequence of moving images.

General appearance and behaviour
The appearance of a patient can reveal a great deal about their mental state, especially to the trained and experienced eye. Key factors, such as chronological age, whether the patient appears their age, their dressing style, and whether they are well-groomed and can maintain eye contact, are some of the behavioural and appearance factors considered.
General appearance and behaviour can help to identify problems with alcohol and substance abuse, among other mental health concerns, such as depression.
Thought content
We also ask direct questions to better determine and assess your train of thought. The questions help to evaluate whether the patient's thoughts are logical and consistent. While answering, our specialists consider factors such as whether you are blocking any thoughts. Does your train of thought make sense?
Through this assessment, we can detect negative feelings such as hopelessness that can point to various mental health conditions, such as depression and others.
Cognitive function
Cognitive function relates to intellectual abilities. This mental health assessment helps check the capacity to read, write and remember significant events in the recent past with ease. We may also use this assessment to determine if the patient can recognise familiar people and places. Cognitive functioning also includes the ability to focus or perform simple calculations.
Judgement and insight
Judgement is determined by how well you can solve problems and make informed choices. Insight is evaluated by gathering as much information as possible from the patient’s perspective. We have various behavioural tests that can help shed more light on your judgement and insight abilities.
